Name of the Contract: «Procurement of medical equipment and consumables for healthcare facilities of the secondary and tertiary levels (phase 2) »

1.         The Ukraine has received financing from the World Bank toward the cost of the Serving People Improving Health Project, and intends to apply part of the proceeds toward payments under the contracts for procurement of medical equipment and consumables for the health facilities of the secondary and tertiary levels. Bidding process will be governed by the World Bank’s rules and procedures.

2.         The Healthcare Department of Volyn Oblast State Administration invites sealed bids from eligible bidders for the following supply of medical equipment and consumables for the health facilities of the secondary and tertiary levels of Volyn oblast on the following positions:

 

Lot No.1: ICB-G 6.2.2.2/1

Cardiac intensive care equipment

1. Anaesthesia machine– 1 pc.

2.Monitor defibrillator   – 1 pc.

3.Syringe pump – 64 pc.

4.Oxygen concentrator – 3 pc.

5. Station for patient’s state monitoring with bedside cardiac monitor (type 1) for 6 patients - 6 pc.

6. Cardiac monitor (type 2) with invasive pressure– 2 pc.

7. Artificial lung ventilation device – 4 pc.

1.Premium digital ultrasound system with the possibility of 3D reconstruction (type 1) – 1 pc.

3.             Bidders may bid for one, several lots, combination of lots, or for all lots as further defined in the bidding document for complete list of goods/services within each lot. Partial bids will not be considered and will be rejected as non-responsive. Evaluation of Bids will be carried out for each lot separately.

Bidders who want to offer discounts in case of award more than one contract, are allowed to do this provided that such discounts will be included in the Letter of Bid

4.             Bidding will be conducted through the International Competitive Bidding (ICB) procedures as specified in the World Bank’s Guidelines: Procurement of Goods, Works, and Non-Consulting Services under IBRD Loans and IDA Credits & Grants dated January 2011 (revised in July 2014) (“Procurement Guidelines”), and is open to all eligible bidders as defined in the Procurement Guidelines. In addition, please refer to paragraphs 1.6 and 1.7 setting forth the World Bank’s policy on conflict of interest.

5.             Interested eligible bidders may obtain further information and inspect the bidding documents during office hours 9:00 to 17:00 hours at the address given below (1) in the office of the Sub-project management unit.

6.      A complete set of bidding documents in English or Ukrainian may be received by interested eligible bidders in PDF soft copy free of charge upon the submission of a written application to the address below (1)

7.             Qualifications requirements include: (i) absence of deferred debts to tax authorities; (ii) At least 5 (five) years of experience in supplying similar goods for each lot; (iii)

Successful completion of at least 2 (two) contracts (for supply of medical/similar equipment equipment) during last 5 (five) years for the amount of each contract that is similar or higher:

Lot 1:420 000,00 USD;

Lot 2:800 000,00 USD;

Lot 3:112 000,00 USD;

Lot 4:300 000,00 USD;

Lot 5:67 000,00 USD;

Lot 6:272 000,00 USD;

Lot 7:400 000,00 USD;

8. Bids must be delivered to the address below (2) before 10:00 local time 17 January 2019. Electronic bidding will not be permitted. Late bids will be rejected. Bids will be publicly opened in the presence of the bidders’ designated representatives and anyone who choose to attend at the address below.

9. All bids must be accompanied by a Bid Security. The amount and currency of the bid security shall be:
