Contract Title: Equipment for microscope diagnostics, for colposcopediagnostics, for invasive examinations and treatment in the Cancer Treatment and Diagnostics Centre, in multi-districtscreening centres, and for gynaecological rooms in the oblast districts.

1. The Ukraine has receivedfinancing from the World Bank toward the cost of the Serving People Improving Health Project 8475-UA, and intends to apply part of the proceeds toward payments under the contract for Equipment for microscope diagnostics, for colposcopediagnostics, for invasive examinations and treatment in the Cancer Treatment and Diagnostics Centre, in multi-districtscreening centres, and for gynaecological rooms in the oblast districts.
Bidders may bid for one or several contracts, as further defined in the bidding document. Bidders wishing to offer discounts in case they are awarded more than one contract will be allowed to do so, provided those discounts are included in the Letter of Bid.
Bidding process will be governed by the World Bank’s rules and procedures.
2. The Health care Department of Lviv Oblast State Administration now invites sealed bids from eligible bidders for the supply of equipment for microscope diagnostics, for colposcope diagnostics, for invasive examinations and treatment in the Cancer Treatment and Diagnostics Centre, in multi-district screening centres, and for gynaecological rooms in the oblast districts as per the following Lots:
Lot №1:
1. Surgical lamp with video monitoring (shadow-free operating-theatre light fixture accompanied by a video system) – 1 Pc.;
2. Surgical lamp (a ceiling-mounted shadow-free operating-theatre light fixture) – 3 Pcs.;
3. Gynaecological chair; type 1 – 3 Pcs.;
4. Gynaecological chair (pneumatic); type 2 – 3 Pcs.;
5. Operating table (universal electrohydraulic with a gynaecological set); type 1 – 1 Pc.;
6. Operating table (mechanical hydraulic); type 2 – 3 Pcs.;
Lot№2:
7. Colposcope (digital colposcope) – 3 Pcs.;
8. Digital colposcope of an expert-class (with a floor tripod designed for zoomed-out stereoscopic image field of view with an extensive depth of field) – 2 Pcs.;
9. Binocular microscope with the option of an integrated computer monitor. (microscope for laboratory testing equipped with a documenting system) – 1 Pc.;
10. Digital binocular microscope (a microscope for laboratory studies); type 1 – 10 Pcs.;
11. Digital binocular microscope; type 2 – 15 Pcs.;
12. Surgical microscope – 1 Pc.;

13. Electrosurgical block; type 1 (universal high-frequency surgical generator with a unit for argon-enhanced electrosurgery (a complete set of 16 pieces) – 1 Pc.;
14. Electrosurgical block; type 2 (universal high-frequency surgical generator with a block for argon-enhanced electrosurgery (a complete set of 14 pieces) – 1 Pc.;
15. Electrosurgical block; type 3 (multi-function electrosurgical device, with dual electric ligation modes for vessels and for tissues) – 1 Pc.;
16. Electrosurgical block; type 4 (universal high frequency surgical generator) – 3 Pcs.

17. Surgical instruments for thoracoscopic lobectomy – 5 Pcs.;
18. A set of video laparoscopic equipment; type 1 (a set of equipment for laparoscopy) – 1 Set;
19. A set of video laparoscopic equipment; type 2 (for thoracoscopy) – 1 Set;
20. A set of video laparoscopic equipment; type 3 (for Urology) – 1 Set;
21. Binocular loupe (magnifying device with a headlamp) – 2 Pcs.;
22. Electrosurgical block; type 5 (electrosurgical and ultrasound generator) – 1 Pc.;
23. Anaesthesia device – 3 Pcs.;
24. Monitoring System; type 1 – 2 Pcs.;
25. Monitoring System; type 2 – 1 Pc.;
Lot №5:
26. Gynaecological sets for taking cervical samples – 164 000 Sets;
27. Cytology brushes for taking cervical samples. (Cytology brushes; type D) – 800 000 Pcs.;.
Qualification requirements include:
If Bidder is Manufacturer: Local bidders shall submit evidence of not having deferred debts to tax authorities; bidders must have at least 5 (five) years of experience in manufacturing similar goods under each lot. The qualification of the Bidders submitting bids for several lots will be assessed on a cumulative basis;
successful completion of at least 2 (two) contracts for the amount for each contract that is similar to or more than for: Lot № 1 – 120,000 (one hundred twenty thousand) U.S. Dollars, Lot №2 – 150,000 (one hundred fifty thousand) U.S. Dollars, Lot №3 – 250,000 (two hundred fifty thousand) U.S. Dollars, Lot №4 – 350,000 (three hundred fifty thousand) U.S. Dollars, Lot №5 – 300,000 (three hundred thousand) U.S. Dollars for supply, installation and technical support of medical equipment during the last 3 (three) years,
If Bidder is not manufacturer:
If a Bidder is not a manufacturer, but is offering the Goods on behalf of the Manufacturer under Manufacturer's Authorization Form (Section IV, Bidding Forms), the Manufacturer shall demonstrate the above qualifications (i), (ii) (1), and the Bidder shall demonstrate that it has successfully completed at least 2 (two) contracts for supply, installation and technical support of similar medical equipment during last 3 (three) years for the amount for each contract that is similar to or more than for: Lot № 1 – 120,000 (one hundred twenty thousand) U.S. Dollars, Lot №2 – 150,000 (one hundred fifty thousand) U.S. Dollars, Lot №3 – 250,000 (two hundred fifty thousand) U.S. Dollars, Lot №4 – 350,000 (three hundred fifty thousand) U.S. Dollars, Lot №5 – 300,000 (three hundred thousand) U.S. Dollars
Bidders may bid for one or several lots, as further defined in the bidding document for complete list of items within each lot only. Partial bids will not be considered and will be rejected as non-responsive. Bidders wishing to offer discounts in case they are awarded more than one contract will be allowed to do so, provided those discounts are included in the Letter of Bid.
3. Bidding will be conducted through the International Competitive Bidding procedures as specified in the World Bank’s Guidelines: Procurement of Goods, Works, and Non-Consulting Services under IBRD Loans and IDA Credits & Grants dated January 2011 (revised in July 2014) (“Procurement Guidelines”), and is open to all eligible bidders as defined in the Procurement Guidelines. In addition, please refer to paragraphs 1.6 and 1.7 setting forth the World Bank’s policy on conflict of interest.
4. Interested eligible bidders may obtain further information from Health care Department of Lviv Oblast State Administration and inspect the bidding documents during office hours 10:00 to 17:00 at the address given below.
5. A complete set of bidding documents in English or Ukrainian may be purchased by interested eligible bidders upon the submission of a written application to the address below and upon payment of a nonrefundable fee of USD 50 or an equivalent amount in Ukrainian Hryvnia. 6. Bids must be delivered to the address below on or before16:00 local timeon May 25, 2016. Electronic bidding will not be permitted.Late bids will be rejected. Bids will be publicly opened in the presence of the bidders’ designated representatives and anyone who choose to attend at the address below.
