Name of the Contract:“Medical equipment for installing at 39 ambulatories”
Reference No. (as per Procurement Plan):ICB No.1.2.1
1. Ukraine has received financing from the World Bank toward the cost of the Serving People Improving Health Project,No.8475-UA , and intends to apply part of the proceeds toward payments under the contract for procurement of medical equipment for installing at 39 ambulatories in the cities of Dnipro and Kryvyi Rih.Bidding process will be governed by the World Bank’s rules and procedures.
2. The Health Care Department of Dnipropetrovsk Oblast State Administration now invites sealed bids from eligible bidders for procurement of medical equipment for installing at 39 ambulatories of Dnipropetrovsk oblast, for the following items:
1. Fingertip pulse oximeter – a device for measuring the level of oxygen in the blood by bloodless method.– 186 pcs.
2. Bilirubinometer.– 39 pcs.
3. Device for measuring sugar level in the blood (glucometer).– 546 pcs.
4. Cholesterol meter.– 39 pcs.
5. ECG-telemetry device.– 186 pcs.
6. Portable three-channel cardiograph with combined power supply and automatic decryption.– 117 pcs.
7. Portable fetal doppler.– 39 pcs.
8. Eye tonometer for measuring intraocular pressure (transpalpebral).– 186 pcs.
9. Medical bag of doctor/nurse.– 546 pcs.
10. Tonometer with cuffs for measuring blood pressure in the arms and legs and baby cuff.– 546 pcs.
11. Portable oto-ophthalmoscope (fundus examination).– 186 pcs.
12. Portable nebulizer, nebulizer kit (mask, mouthpiece, camera). – 186 pcs.
3. Bidders shall submit their bids as per the Bidding Documents regarding the complete list of items. Partial bids will not be considered and will be rejected as non-responsive.
4. Bidding will be conducted through the International Competitive Bidding procedures (ICB) as specified in the World Bank’s Guidelines: Procurement of Goods, Works, and Non-Consulting Services under IBRD Loans and IDA Credits & Grants, dated January 2011 (revised in July 2014) (“Procurement Guidelines”), and is open to all eligible bidders as defined in the Procurement Guidelines.Besides, in paragraphs 1.6 and 1.7 of the Procurement Guidelines, the World Bank’s policy on the conflict of interests is set out.
5. Interested eligible bidders may obtain further information and inspect the bidding documents during office hours 10:00 to 17:00 at the address given below (1).
6. A complete set of Bidding Documents in English or Ukrainian may be received by interested bidders for free subject to submission of a written Application to the address below (1).Authorized representatives of the Bidders may receive the Bidding Documents at the office of Sub-Project Management Unit at the address below (1) or by method of courier delivery upon condition that this service shall be made payable by the appropriate bidder directly to the providing delivery agents.
7. Qualification requirements include: (i) unavailability of overdue debts to the tax authorities; (ii) at least five (5) years of experience in manufacturing similar products; (iii) successful completion of at least two (2) contracts for the supply, installation and technical support of medical equipment during the previous five (5) years in the amount of the contract, which is similar or more than: in amount not less than $1 250 000.00.

8. Bids must be delivered to the below address (1) before 11 am local timeOctober18,2016.Electronic bidding will not be permitted.Late bids will be rejected.Bids will be publicly opened in the presence of the bidders’ designated representatives, who choose to attend at the address below(2) on11:15am local timeOctober18,2016.
9. Bids must be supplemented with Bid Security.Amount and currency of the Bid Security are as follows:
16 000 (sixteen thousand) U.S. Dollarsor equivalent in the currency of the bid at the exchange rate of the National Bank of Ukraine (www.bank.gov.ua) on the date 14 (fourteen) days prior to bid submission.
