Contract Title: Test reagents, laboratory equipment, and glassware
Reference No. (as per Procurement Plan): № ICB 7.1.2
1. The Ukraine has received financing from the World Bank toward the cost of the Serving People Improving Health Project, No.8475-UAand intends to apply part of the proceeds toward payments under the contracts for Test reagents, laboratory equipment, and glassware. Bidding process will be governed by the World Bank’s rules and procedures.
2. The Health Department of Poltava Oblast State Administrationnow invites sealed bids from eligible bidders for the following Test reagents, laboratory equipment, and glassware:
LOT 1:
1.Glucometer-37 pcs.
2.Test strips for glucometers – 400 000 pcs.
3. Urine analyzer semi-automatic-37 pcs.
4. Test strips for urine analyzer – 400 000 pcs.
LOT2:
1. Cooler bag with icepacks – 1890 pcs.
2. Medical Specimen Bag – 1890 pcs.
3. Systems for vacuum blood collection - 520 000 sets
4. Urine container - 520 000pcs.
LOT3:
1. Centrifuge – 945 pcs.
2. Binocular Microscope – 37 pcs.
LOT4:
1. Biochemical analyzer-71 pcs.
2. Reagents for biochemical analyzer (Kit 1) – 400 000 sets.
3. Reagents for biochemical analyzer (Kit 2) – 120 000 sets.
4. Immunochemistry analyzer – 1 pc.
5. Reagents for Immunochemistry analyzer – 1 200 sets.
LOT5:
1. Hematology analyzer-37 pcs.
2. Reagents for Hematology analyzer – 400 000 tests
LOT6:
1. Coagulation analyzer semi-automatic – 37 pcs.
2. Consumables and Reagents for coagulation analyzer – 400 000 tests
3. Aggregometer – 37 pcs.
4. Reagents for aggregometer – 120 000 sets
5. Electrolyte Analyzer – 37 pcs.
6. Consumables and Reagents for Electrolyte Analyzer- 120 000 sets
3. Bidders may bid for one or several lots, as further defined in the bidding document for complete list of items within each lot only. Partial bids will not be considered and will be rejected as non-responsive.Bidders wishing to offer discounts in case they are awarded more than one contract will be allowed to do so, provided those discounts are included in the Letter of Bid.
4. Bidding will be conducted through the International Competitive Bidding procedures as specified in the World Bank’s Guidelines: Procurement of Goods, Works, and Non-Consulting Services under IBRD Loans and IDA Credits & Grants dated January 2011 (revised in July 2014) (“Procurement Guidelines”), and is open to all eligible bidders as defined in the Procurement Guidelines. In addition, please refer to paragraphs 1.6 and 1.7 setting forth the World Bank’s policy on conflict of interest.
5. Interested eligible bidders may obtain further information and inspect the bidding documents during office hours 10:00 to 17:00 hoursat the address given below.
6. A complete set of bidding documents in English or Ukrainian may be purchased by interested eligible bidders upon the submission of a written application to the address below and upon payment of a nonrefundable fee of USD 50,00 oran equivalent amount in Ukrainian Hryvnia at the exchange rate of the National Bank of Ukraine(http://www.bank.gov.ua) on the date of payment. The method of payment will be direct transfer to specified banking account:

All banking charges and fees related to the bank transfer shall be for bidder’s account.
The document will be sent by mail/airmail or handed to the Bidder’s representative at specified address.
7. Qualifications requirements include: (i) absence of deferred debts to tax authorities; (ii) At least five (5) years of experience in manufacturing similar goods for each lot; (iii) Successful completion of at least 2 (two) contracts for supply, installation and technical support of medical equipment during last 5(five) years for the amount for each contract that is similar to or more than:
Lot 1: in amount not less than 640,000 US Dollars,
Lot 2: in amount not less than 800,000 US Dollars
Lot 3: in amount not less than 720,000 US Dollars
Lot 4: in amount not less than 4,960,000 US Dollars
Lot 5: in amount not less than 1,840,000 US Dollars
Lot 6: in amount not less than 960,000 US Dollars
Qualification of the Bidders submitting bids for several lots will be assessed on a cumulative basis.

8. Bids must be delivered to the address below on or before 12:00amlocal time
on November 9th, 2016.Electronic bidding will not be permitted.Late bids will be rejected. Bids will be publicly opened in the presence of the bidders’ designated representatives and anyone who choose to attend at the address below.
9. All bids must be accompanied by a Bid Security. The amount and currency of the bid security shall be:
LOT 1: 12000 (twelve thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date.
LOT 2: 15000 (fifteen thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
LOT 3: 13000 (thirteen thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
LOT 4: 93000 (ninety-three thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
LOT 5: 34 000 (thirty-four thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
LOT 6: 18000 (eighteen thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
