Contract Title: Procurement of equipment as per the PHC center outfit list
Reference No. (as per Procurement Plan): ICB 2.1.3
1. The Ukraine has received financing from the World Bank toward the cost of the Serving People Improving Health Project, No.8475-UAand intends to apply part of the proceeds toward payments under the contracts for Procurement of equipment as per the PHC center outfit list. Bidding process will be governed by the World Bank’s rules and procedures.
2. The Health Department of Zaporizhzhia Oblast State Administrationnow invites sealed bids from eligible bidders for the following Procurement of equipment as per the PHC center outfit list:
LOT 1:
1. Sphygmomanometer with set of cuffs - 1807 pcs.
2. Stethoscope - 1518 pcs.
3. Blood pressure measurement cuff for leg (thigh cuff) - 275pcs.
4. Medical bag - 1602pcs.
LOT 2:
1. Otoscope-/Ophthalmoscope Set – 950 pcs.
LOT 3:
1. Electronic medical scale - 603 pcs.
2. Height meter - 578pcs.
3. Girth Measuring Tape - 2638 pcs.
LOT4:
1. Blood Glucose Monitoring System (Glucometer) - 2354 pcs.
LOT5:
1. ECG machine - 380 pcs.
LOT6:
1. Pulse oximeter - 1000 pcs.
All bids shall be supported by:
If Bidder is Manufacturer:
Local bidders should submit evidence of not having deferred debts to tax authorities.
Bidders must have at least five (5) years of experience in manufacturing similar goods for each lot;
The qualification of the Bidders submitting bids for several lots will be assessed for each lot separately.
Successful completion of at least 2 (two) contracts similar to the proposed during last 5 (five) years for the amount for each contract:
Lot 1: in amount not less than 160,000 (one hundred sixty thousand) U.S. Dollars,
Lot 2: in amount not less than 190,000 (one hundred ninety thousand) U.S. Dollars
Lot 3: in amount not less than 300,000 (three hundred thousand) U.S. Dollars
Lot 4: in amount not less than 380,000 (three hundred eighty thousand) U.S. Dollars
Lot 5: in amount not less than 450,000 (four hundred fifty thousand) U.S. Dollars
Lot 6: in amount not less than 70,000 (seventy thousand) U.S. Dollars
Qualification of the Bidders submitting bids for several lots will be assessed on a cumulative basis.
For confirmation Bidders must provide copies of such contracts and feedback about their performance for the period last 5 (five) years.
Documentary evidence are provided in a form of a link to the main contracts for the supply and technical support for medical equipment, which the Bidder has successfully completed during the period since 2011. List of major contracts should contain information about such contracts, including reference to the contract number, the total quantity and type of goods, delivery date, name of the Purchaser, address and telephone/fax.
If Bidder is not manufacturer:
If a Bidder is not a manufacturer, but is offering the Goods on behalf of the Manufacturer under Manufacturer's Authorization Form (Section IV, Bidding Forms), the Manufacturer shall demonstrate the above qualifications a(i), (ii)(1) and the Bidder shall demonstrate that it has successfully completed at least 2 (two) contracts of similar medical equipment during last 5 (five) years for the amount for each contract:
Lot 1: in amount not less than 160,000 (one hundred sixty thousand) U.S. Dollars,
Lot 2: in amount not less than 190,000 (one hundred ninety thousand) U.S. Dollars
Lot 3: in amount not less than 300,000 (three hundred thousand) U.S. Dollars
Lot 4: in amount not less than 380,000 (three hundred eighty thousand) U.S. Dollars
Lot 5: in amount not less than 450,000 (four hundred fifty thousand) U.S. Dollars
Lot 6: in amount not less than 70,000 (seventy thousand) U.S. Dollars
Bidders may bid for one or several lots, as further defined in the bidding document for complete list of items within each lot only. Partial bids will not be considered and will be rejected as non-responsive.Bidders wishing to offer discounts in case they are awarded more than one contract will be allowed to do so, provided those discounts are included in the Letter of Bid.
3. Bidding will be conducted through the International Competitive Bidding procedures as specified in the World Bank’s Guidelines: Procurement of Goods, Works, and Non-Consulting Services under IBRD Loans and IDA Credits & Grants dated January 2011 (revised in July 2014) (“Procurement Guidelines”), and is open to all eligible bidders as defined in the Procurement Guidelines. In addition, please refer to paragraphs 1.6 and 1.7 setting forth the World Bank’s policy on conflict of interest.
4. Interested eligible bidders may obtain further information and inspect the bidding documents during office hours 10:00 to 17:00 hoursat the address given below.
5. A complete set of bidding documents in English or Ukrainian may be purchased by interested eligible bidders upon the submission of a written application to the address below and upon payment of a nonrefundable fee of USD 50.00 (fifty) oran equivalent amount in Ukrainian Hryvniaatthe exchange rate of the National Bank of Ukraine(www.bank.gov.ua) on the date of payment. The method of payment will be direct transfer to specified banking account:

The document will be sent by mail/airmail or handed to the Bidder’s representative at specified address.
6. Bids must be delivered to the address below on or before 15:00 local time
on December 19, 2016.Electronic bidding will not be permitted.Late bids will be rejected. Bids will be publicly opened in the presence of the bidders’ designated representatives and anyone who choose to attend at the address below.
7. All bids must be accompanied by a Bid Security. The amount and currency of the bid security shall be:
LOT 1: 3000 (three thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date.
LOT 2: 3000 (three thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
LOT 3: 4000 (four thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
LOT 4: 6000 (six thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
LOT 5: 7000 (seven thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
LOT 6: 1000 (one thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date.
Bid Security shall be in the form of irrevocable bank guarantee. All other forms of securities are unacceptable by the Purchaser.
