Medical equipment and medical products to equip interventional cardiology department in the city of Khust and ZOCCH
Reference No.: ICB 3.1.7.
1. The Government of Ukraine has received a loan from the World Bank toward the cost of the “Improving health in the service of the people”, №8475-UA Project and intends to apply part of the proceeds toward payments under the contract for procurement of Medical equipment for equipping general practice clinics and medical equipment for equipping structural units of emergency medical aid. Medical equipment and medical products to equip interventional cardiology department in the city of Khust and ZOCCH (package 3.1.7.)
Bidding will be conducted according to the rules and procedures of the World Bank.
2. The Healthcare department of Zakarpattia Oblast State Administration now invites sealed bids from eligible bidders for the supply of the following goods:
Lot 1:

“3-channel ECG monitor with multigas modules” – 3pieces.

“3-channel ECG monitor”- 16 pieces.

“Central monitoring station” – 1 piece.

“Defibrillator” – 30 pieces.

“Defibrillator with advanced features” – 4 pieces.

“ECG recording device (Type 1)”- 2 pieces.

“ECG recording device (Type 2)”- 204 pieces.

« Defibrillator with electrodes for open heart defibrillation » – 1 piece.
Lot 2

«Electrosurgery generator monopolar, bipolar, complete with accessories» - 1 piece.

“Centrifugal blood pump”- 1 piece.

“Ultrasonic dissector” – 1 piece.

“Intra-aortic balloon pump” – 1 piece.

“Ventilator with compressor” – 4 pieces.
Lot 3

“Device for measuring coronary blood flow fractional flow reserve (FFR + OST)” - 1 piece.

“Activated Clotting Time measurement device” – 1 piece.
Lot 4

“Autologous Blood Recovery System»-1 piece.
Bidders shall meet the following qualification criteria:
(1) Presence of at least 5 years of experience in manufacturing of equipment
The qualification of bidders who submit bids for several lots will be evaluated separately for each lot
(2) Successful completion of at least two (2) contracts similar to the offered, cost of each:
for lot 1 - must be equivalent or more than the equivalent of 500 000 USD during the last 3 years
for lot 2 - must be equivalent or more than the equivalent of 300 000 USD during the last 3 years;
for lot 3 - must be equivalent or more than the equivalent of 300 000 USD during the last 3 years;
for lot 4 - must be equivalent or more than the equivalent of 30 000 USD during the last 3 years;
Margin of preference shall not apply.

3. Bidders may bid for one or several contracts, as further defined in the bidding document. Bidders wishing to offer discounts in case they are awarded more than one contract will be allowed to do so, provided those discounts are included in the Letter of Bid.
4. Bidding will be conducted through the International Competitive Bidding procedures as specified in the World Bank’s “Guidelines: Procurement of Goods, Works and Non-ConsultingServices under IBRD Loans and IDA Credits & Grants by World Bank Borrowers” published in January 2011 and revised in July 2014 (“Procurement Guidelines”), and is open to all eligible bidders as defined in the Procurement Guidelines. In addition, please refer to paragraphs 1.6 and 1.7 setting forth the World Bank’s policy on conflict of interest.
5. Interested eligible bidders may obtain further information in Healthcare department of Zakarpattia oblast state administration, contact person Ivan Hushtan, deputy Chairman of the Selection Committee or Natalya Roshko, secretary of the the Selection Committee and inspect the bidding documents during office hours from 8:00 to 17:00 at the address given below office 201-203, Narodna square 4, Uzhhorod, Ukraine,88008.
6. A complete set of bidding documents may be purchased by interested eligible bidders upon the submission of a written application to the address below and upon payment of a nonrefundable fee of 1250,00 UAH or 50 USD. The method of payment will be clearing settlement. Documentation can be obtained at the office of the Project Consultancy Support Unit at presence of an appropriate errand for the bidders' representatives and a copy of payment document or may be sent by courier to the address indicated in the written application submitted by the Bidder together with the copy of the payment document.
The method of payment should be a direct money transfer to the specified account:

For USD payment: account # 26000000015423 at the Uzhhorod branch of PJSC "State export-import bmk of Ukraine", MFO local banking code 322313, USREOU code 02012852.
All bank fees are charged to the bidder.
7. Bids must be delivered to the address below or before 08 November 2017, 11.00 a.m. local time. Electronic bidding will not be permitted. Late bids will be rejected. Bids will be publicly opened in the presence of the bidders’ designated representatives and anyone who choose to attend at 08 November 2017, 11.00 a.m. local time.
8. All bids must be accompanied by a Bid Security of amount:
Lot 1: 7 500,00 (seven thousand five hundred) USD or equivalent in currency of the bid by exchange rate indicated in Financial Time website (http://markets.ft.com/Research/Markets/Data-Archive) on the date 14 (fourteen) days before the bid submission date.
Lot 2: 3 500,00 (three thousand five hundred) USD or equivalent in currency of the bid by exchange rate indicated in Financial Time website (http://markets.ft.com/Research/Markets/Data-Archive) on the date 14 (fourteen) days before the bid submission date.
Lot 3: 3 800,00 (three thousand eight hundred) USD or equivalent in currency of the bid by exchange rate indicated in Financial Time website (http://markets.ft.com/Research/Markets/Data-Archive) on the date 14 (fourteen) days before the bid submission date.
Lot 4: 500,00 (five hundred) USD or equivalent in currency of the bid by exchange rate indicated in Financial Time website (http://markets.ft.com/Research/Markets/Data-Archive) on the date 14 (fourteen) days before the bid submission date.
