TERMS OF REFERENCE
on providing consultant services (individual consultant)
of
“Consultant Trainer in the Field of Cardiology”
(Procurement package No. 7.3.2)
Project: Serving People, Improving Health
World Bank Loan no. 8475-UA
Subproject: “Introducing an innovative model of providing services to patients with hypertension in Poltava region”
I. Project Background
The Government of Ukraine has received the loan from the World Bank to fund “Serving People, Improving Health” project (hereafter — the Project).The goals of the Project are improvement of the quality of healthcare services in selected oblasts with a special accent on primary and secondary prevention of cardiovascular diseases and cancer, and improvement of the efficiency of healthcare system.
The Project comprises of the following parts:
Part 1. Improving service delivery at the local level.
Part 2. Strengthening MoH governance.
Part 3. Project implementation support, and monitoring and evaluation.
Under Part 1 of the Project, Poltava oblast was provided with funding to perform the investment subproject approved by the World Bank and aimed at prevention, early detection, and treatment of cardiovascular diseases or complications thereof, as well as increasing the efficiency of healthcare provision systems, including measures focused on improvement of primary care, fighting cardiovascular diseases on the levels of primary, secondary, and tertiary care.
To ensure the effective implementation of “Improving service delivery at the local level” in Poltava Oblast, the Oblast State Administration creates the Subproject Management Unit (hereafter — the SMU).
Department of Health of the OSA (hereinafter referred to as the Client) intends to engage consultant trainers (hereinafter each of them referred to as the Consultant) to train trainer doctors under these Terms of Reference.
II. Objective of the Assignment
The objective is to develop a curriculum and content of 8 training and simulation modules covering modern methods of hospital care for cardiovascular diseases and complications thereof, and to use the modules to train 35 trainers from among medical personnel of HCFs of Poltava oblast.
After the training, the above medical personnel (hereafter — the Trainers) shall conduct interdisciplinary and interprofessional trainings with medical personnel (doctors and nurses) based on the 8 training modules covering certain topics, and have training and methodological materials to conduct such trainings.
III. Scope of Services
3.1. The list of topics of the training simulation modules for medical personnel of secondary and tertiary care facilitiesof Poltava oblast:
1. Hospital care for hypertensive crises (7 hours).
2. Hospital care for ST-elevation acute coronary syndrome (7 hours).
3. Hospital care for non-ST-elevation acute coronary syndrome (7 hours).
4. Hospital care for sudden coronary or cardiac death (7 hours).
5. Hospital care for cerebrovascular accidents (7 hours).
6. Hospital care for fatal paroxysmal arrhythmia or bradyarrhythmia (7 hours).
7. Hospital care for cardiac asthma and pulmonary oedema (7 hours).
8. Hospital care for PATE (7 hours).
3.2.Curriculum and content of a training simulation module.Teaching and learning materials fro a training and simulation module shall include:
(a) lecture materials in Power Point format prepared according to international standards and existing regulatory framework — national standards;
(b) tests to evaluate baseline knowledge level and the level of retention of theoretical material;
(c) case studies regarding standard clinical cases;
(d) case studies regarding rare clinical cases;
(e) algorithms of performing practical skills.
(f) scenarios of simulation trainings (at least 5)
(g) algorithms of holding a debriefing session
(h) clinical paths of patients
(i) local protocols and standards
3.3. Structure for a one-topic training and simulation module.
StageI.Online training course (lecture).Audience size: up to 300 persons.
Stage II.Conducting an interprofessional and interdisciplinary simulation training for Trainers that must include the following components:
a) baseline knowledge evaluation;
b) organizing teams;
c) practical simulation session using models/simulators for each team;
d) debriefing session;
e) final test and discussion.
The number of trainings for Trainers within one training and simulation module — 3. The number of trainers trained — 35.
Stage III.Consulting Trainersincludes:
a) consulting trainer doctors on preparation to conducting their trainings;
b) support and counselling during Trainers’ conducting an interprofessional and interdisciplinary simulation training.The number of Trainers whose trainings were evaluated by the Consultant Trainer shall be at least 17 persons.
c) analysis of the way the Trainers’ trainings were conducted and providing respective recommendations.
Stage IV.Preparation of a reportaccording to a template
IV. Duration and Place of Work
The Consultant shall provide services starting from the Contract conclusion date until __ ____ 20__, or during another period agreed upon by the parties at the time of Contract conclusion.
The place of work will be the city of Poltava, Ukraine.Business trips in Poltava oblast are possible up to 5 times a month.
V. Implementation Arrangements
The Consultant, while performing his/her functions, shall be subordinated to the head of the Department of Health of the OSA.
A contract providing for hourly payment will be signed with the Consultant (standard contract form of the World Bank).The Consultant shall work on the hourly basis.Consultant's services shall be paid for in hryvnia, monthly, according to the official exchange rate as of the date of signing of the Service Acceptance Act.
The Customer shall provide the Consultant with the workplace, including the equipment required to perform the job (phone, fax, PC with Internet access).
The Customer shall also provide the Consultant with information required to perform the tasks.
Within 10 days upon the end of each month, the Consultant shall submit reports on the services provided in the format determined by the Customer.The reports shall be prepared so as to be readable and understandable for the Customer.
VI. Qualification Requirements
Consultant’s qualification shall meet the following minimum requirements:
Mandatory requirements:
(a) a university degree in medicine;
(b) having all practical skills corresponding to the topics of the training and simulation modules;
(c) min. 5 years of work experience as a doctor;
(d) fluent Ukrainian;
(e) computer and video equipment skills (Microsoft Windows, Word);
(f) public speaking skills (lectures, reports) supported by some documents;
(g) experience of conducting trainings for medical personnel supported by documentary evidence;
(h) skills of preparing lectures in Power Point, as well as case studies and tests;
(i) understanding the principles of training organization and conducting;
(j) ability to model standard and rare clinical cases;
(k) ability to analyze and discuss correctness of completing the tasks in simulated cases;
(l) skills of work with models, simulators, etc.
Preferrable:
(a) ability of preparing scenarios of simulation trainings;
(b)working level of English.
