E-declarations in the eHealth system using the Doctor Eleks program

As part of the joint World Bank's and the Ministry of Health of Ukraine "Serving People, Improving Health" Project, specifically within the "Preventive oncology and the implementation of effective anti-tumor technologies in Lviv oblast" Subproject, Doctor Eleks software has been delivered to all family outpatient units and antenatal clinics; in the framework of the software, it is planned that the Electronic Registry of the Female Population in Lviv Oblast be introduced.
 
Taking into account the high excitement about the electronic declarations, we convey the following information to all family physicians and to the staff of the primary level:
 
In the Doctor Eleks program, which has been installed in each outpatient unit on a personal computer with a printer that was delivered in 2017, one may enter data for all patients, regardless of gender, age, diagnosis or the reason of their visit;
 
the Doctor Eleks program makes it possible to create and print the following forms of documents and reports approved by the orders of the Ministry of Health: Form 025 / o "Outpatient Patient Medical Card", Mandatory Medical Preventive Examination of a Child, Form 028 / o "Advisory Conclusion", Form 027 / o "Excerpt from the medical card of an outpatient patient", Form 058 / o "Emergency notification", Form 090 / o "Reporting on a patient who was diagonsed with cancer for the first time", Form 095 / o "Certificate of temporary disability of a student", Form 112 / о History of development of a child, form 063 / a Map of preventive immunization, form 074 / a Registry log of outpatient patients etc.
 
Through the Doctor Eleks program one may create eHealth declarations.
 
In addition, we would like to inform that for the purpose of creating a declaration in the eHealth system through the Doctor Eleks program, it is recommended that one registers your institution and a doctor in eHealth through https://doctor.eleks.com/ehealth/.
