Contract Title: Equipment for radiological diagnostic methods in the Сancer Treatment and Diagnostics Centre and multi-regional screening centres.

Reference No. (as per Procurement Plan):ICB4.1.2

1. The Ukraine has receivedfinancing from the World Bank toward the cost of the Serving People Improving Health Project 8475-UA, and intends to apply part of the proceeds toward payments under the contract for Equipment for radiological diagnostic methods in the Сancer Treatment and Diagnostics Centre and multi-regional screening centres.

Bidders may bid for one or several contracts, as further defined in the bidding document. Bidders wishing to offer discounts in case they are awarded more than one contract will be allowed to do so, provided those discounts are included in the Letter of Bid.Bidding process will be governed by the World Bank’s rules and procedures.

2. The Health care Department of Lviv Oblast State Administration now invites sealed bids from eligible bidders for the supply of Equipment for radiological diagnostic methods in the Сancer Treatment and Diagnostics Centre and multi-regional screening centres as per the following Lots:

Lot №1:

1. Ultrasound devices of an upper class for carrying out general examinations (Ultrasound devices for diagnostic system with a set of transducers for examining organs of abdominal cavity, small parts, and the intercavitary [intra-abdominal] organs using biopsy needle guides). – 4 Pcs.;

2. Ultrasound devices of an upper class for diagnostic system for General imaging, Cardiology and Elastography.– 2 Pcs.;

2.1. Ultrasound device for diagnostic system for General imaging and Cardiology;

2.2. Ultrasound device for diagnostic system for General imaging and Elastography.

Lot№2:

3. Stand-mounted ultrasound device of an expert class for volumetric scanning of the breast (Ultrasound device for diagnostic system with a set of transducers for examining organs of abdominal cavity, small parts, gynaecology, and urology using a console for volumetric scanning of the breast and a workstation for data processing). – 1 Pc.;

Lot №3:

4. CT (computed tomography) scanner. – 1 Pc.;

5. SPECT/CT system – 1 Pc.;

6. MRI scanner – 1 Pc.;

Lot №5:

7. Digital mammography device of an expert class (mammography system with biopsy and tomosynthesis). – 1 Pc.;

8. Digital mammography device of an expert class (mammography with stereotactic biopsy system). – 2 Pcs.;

9. Digital mammography devices of an upper-class (digital mammography system). – 5 Pcs.;

Lot №6:

10. Digital X-ray diagnostic devices– 2 Pcs.:

10.1. DigitalX-ray system;

10.2. Digital fluoroscopy system.

11. C-arm mobile X-ray device (C-arm X-ray system). – 1 Pc.

Qualification requirements include:

If Bidder is Manufacturer: Local bidders shall submit evidence of not having deferred debts to tax authorities; bidders must have at least 5 (five) years of experience in manufacturing similar goods under each lot; The qualification of the Bidders submitting bids for several lots will be assessed on a cumulative basis;

successful completion of at least 2 (two) contracts for the amount for each contract that is similar to or more than for

Lot № 1 – 600 000(sixhundred thousand) U.S. Dollars,

Lot №2 – 200 000 (two hundred thousand) U.S. Dollars,

Lot №3 – 1 300 000 (one million three hundred thousand) U.S. Dollars,

Lot №4 – 1 500 000(one million fivehundred thousand) U.S. Dollars,

Lot №5 – 1 500 000 (one million five hundred thousand) U.S. Dollars,

Lot №6 – 780 000 (seven hundred eighty thousand) U.S. Dollars, for supply, installation and technical support of medical equipment during the last 3 (three) years.

If Bidder is not manufacturer:

If a Bidder is not a manufacturer, but is offering the Goods on behalf of the Manufacturer under Manufacturer's Authorization Form (Section IV, Bidding Forms), the Manufacturer shall demonstrate the above qualifications (i), (ii) (1), and the Bidder shall demonstrate that it has successfully completed at least 2 (two) contracts for supply, installation and technical support of similar medical equipment during last 3 (three) years for the amount for each contract that is similar to or more than:

Lot № 1 – 600 000(sixhundred thousand) U.S. Dollars,

Lot №2 – 200 000 (two hundred thousand) U.S. Dollars,

Lot №3 – 1 300 000 (one million three hundred thousand) U.S. Dollars,

Lot №4 – 1 500 000(one million fivehundred thousand) U.S. Dollars,

Lot №5 – 1 500 000 (one million five hundred thousand) U.S. Dollars,

Lot №6 – 780 000 (seven hundred eighty thousand) U.S. Dollars,

Bidders may bid for one or several lots, as further defined in the bidding document for complete list of items within each lot only. Partial bids will not be considered and will be rejected as non-responsive. Bidders wishing to offer discounts in case they are awarded more than one contract will be allowed to do so, provided those discounts are included in the Letter of Bid.

3. Bidding will be conducted through the International Competitive Bidding procedures as specified in the World Bank’s Guidelines: Procurement of Goods, Works, and Non-Consulting Services under IBRD Loans and IDA Credits & Grants dated January 2011 (revised in July 2014) (“Procurement Guidelines”), and is open to all eligible bidders as defined in the Procurement Guidelines. In addition, please refer to paragraphs 1.6 and 1.7 setting forth the World Bank’s policy on conflict of interest.

4. Interested eligible bidders may obtain further information from Health care Department of Lviv Oblast State Administration and inspect the bidding documents during office hours 10:00 to 17:00 at the address given below.

5. A complete set of bidding documents in English or Ukrainian may be purchased by interested eligible bidders upon the submission of a written application to the address below and upon payment of a nonrefundable fee of USD 50 or an equivalent amount in Ukrainian Hryvnia. 6. Bids must be delivered to the address below on or before10:00 local timeon May 25, 2016. Electronic bidding will not be permitted.Late bids will be rejected. Bids will be publicly opened in the presence of the bidders’ designated representatives and anyone who choose to attend at the address below.

7. All bids must be accompanied by a Bid Security. The amount and currency of the bid security shall be:

Bid Security shall be in the form of irrevocable bank guarantee. All other forms of securities are unacceptable by the Purchaser.
