Contract Title: Software "Electronic Registry of the female population in Lviv oblast", computer hardware and server equipment
Reference No. (as per Procurement Plan): NCB 4.1.4
1. The Ukraine has receivedfinancing from the World Bank toward the cost of the Serving People Improving Health Project 8475-UA, and intends to apply part of the proceeds toward payments under the contract for Software "Electronic Registry of the female population in Lviv oblast", computer hardware and server equipment.Bidding process will be governed by the World Bank’s rules and procedures.
2. The Health care Department of Lviv Oblast State Administration now invites sealed bids from eligible bidders for Software "Electronic Registry of the female population in Lviv oblast", computer hardware and server equipmentas per the following goods:
1. Software “Electronic Registry of the female population in Lviv oblast” (Server license with unlimited number of working places) – 1 unit;
2. The system unit (Desktop) – Standard working station - 127 units;
3. The system unit (Desktop) – DICOM working station - 28 units;
4. Standard keyboard ukr/rus/eng – 159 units;
5. USB mouse with a scroll wheel – 159 units;
6. Patch cable RJ-45 – 2 meters – 165 units;
7. LED Monitor-– Standard working station – 131units;
8. LED Monitor—DICOM working station – 28 units;
9. An uninterruptible power supply, 3000 W – 1 unit;
10. An uninterruptible power supply, 1000 W – 4 units;
11. An uninterruptible power supply, 500 W – 155 units;
12. Laser printer – 155 units;
13. The local PACS-server - multi-district screening centres – 4 units;
14. The server for database and applications - Lviv state oblast-level cancer treatment and diagnostics centre – 1 unit;
15. The backup server - Lviv state oblast-level cancer treatment and diagnostics centre – 1 unit;
16. PACS server- Lviv state oblast-level cancer treatment and diagnostics centre – 1 unit;
17. Router- Lviv state oblast-level cancer treatment and diagnostics centre – 1 unit;
18. Server access module (KVM console)- Lviv state oblast-level cancer treatment and diagnostics centre – 1 unit;
19. Open frame 19” rack- Lviv state oblast-level cancer treatment and diagnostics centre – 1 unit;
20. Operating system for the server – 7 units;
21. Server software with installed live updates – 7 units;
22. Database management system for server– 2unit;
23. Software package for working with network communications using the DICOM protocol – 5 units;
24. Antivirus software for servers – 7 units;
25. Operating system Windows OS – 155 units;
26. Antivirus software for working station – 155 units.
Qualification requirements include:
If Bidder is Manufacturer:
Local bidders shall submit evidence of not having deferred debts to tax authorities; bidders must have at least 5 (five) years of experience in software development/manufacturing computer equipment similar to those under this procurement package;
Successful completion of at least 2 (two) contracts for the amount of 5 500 000 (five million five hundred thousand) UAH for each contract for software development / computer equipment manufacturing, supply, installation and technical support during the last 2 (two) years;
If Bidder is not manufacturer:
If a Bidder is not a manufacturer, but is offering the Goods on behalf of the Manufacturer under Manufacturer's Authorization Form (Section IV, Bidding Forms), the Manufacturer shall demonstrate the above qualifications a(i), a(ii)(1) and the Bidder shall demonstrate that it has successfully completed at least 2 (two) contracts for the amount of 5 500 000 (five million five hundred thousand) UAH for each contract for software implementation and/or supply of computer equipment, supply, installation and technical support in the past 5 (five) years.
3. Bidding will be conducted through the National Competitive Bidding procedures as specified in the World Bank’s Guidelines: Procurement of Goods, Works and Non-Consulting Services under IBRD Loans and IDA Credits &Grants by World Bank Borrowers(“Procurement Guidelines” January 2011, revised July 2014), and is open to all eligible bidders as defined in the Procurement Guidelines. In addition, please refer to paragraphs 1.6 and 1.7 setting forth the World Bank’s policy on conflict of interest.
4. Interested eligible bidders may obtain further information from The Health care Department of Lviv Oblast State Administration and inspect the bidding documents during office hours 10:00 to 17:00 at the address given below.
5. A complete set of bidding documents in Ukrainian may be purchased by interested eligible bidders upon the submission of a written application to the address below and upon payment of a nonrefundable fee of UAH 1000. The method of payment will be direct transfer to specified banking account:

All banking charges and fees related to the bank transfer shall be for bidder’s account.
The document will be sent by mail/airmail or handed to the Bidder’s representative at specified address.
6. Bids must be delivered to the address below 11:00 local time on August 30, 2016.Electronic bidding will not be permitted.Late bids will be rejected. Bids will be publicly opened in the presence of the bidders’ designated representatives and anyone who choose to attend at the address below.
7. All bids must be accompanied by a Bid Security.
The amount and currency of the bid security shall be:

Bid Security shall be in the form of irrevocable bank guarantee. All other forms of securities are unacceptable by the Purchaser.
