Contract Title: Equipment for laboratory diagnostics in the Cancer Treatment and Diagnostics Centre and multi-regional screening centres.
Reference No. (as per Procurement Plan):ICB4.1.3
1.The government of Ukraine has receivedfinancing from the World Bank toward the cost of the Serving People Improving Health Project 8475-UA, and intends to apply part of the proceeds toward payments under the contract for equipment for laboratory diagnostics in the Cancer Treatment and Diagnostics Centre and multi-regional screening centres.Bidding process will be governed by the World Bank’s rules and procedures.
2. The Health care Department of Lviv Oblast State Administration now invites sealed bids from eligible bidders for the supply of equipment for laboratory diagnostics in the Cancer Treatment and Diagnostics Centre and multi-regional screening centres as per the following Lots:
Lot №1:
1. Device for cryostat specimen staining – 1 Pc.;
2. Device for automatic specimen staining – 1 Pc.;
3. A set of staining reagents for a slide staining station (for 300 000 slides) - 1 Set;
4. Accessories kit for Cryostat – 1 Set;
5. System for express-diagnostics – 1 Pc.
Lot №2:
6. Equipment for the preparation and staining of smears during cytological examinations with a set of reagents and consumables for 9600 tests – 1 Kit;
Lot №3:
7. Automatic hematology analyzer – 1 Pc.
Lot №4:
8. Automatic biochemistry analyzer – 1 Pc.
All bids shall be supported by:
If Bidder is Manufacturer: Local bidders shall submit evidence of not having deferred debts to tax authorities; bidders must have at least 5(five) years of experience in manufacturing similar goods under each lot; The qualification of the Bidders submitting bids for several lots will be assessed on a cumulative basis;
successful completion of at least 2 (two) contracts for the amount for each contract that is similar to or more than for
Lot № 1 – 100 000(onehundred thousand) U.S. Dollars;
Lot №2 – 130 000 (one hundred thirty thousand) U.S. Dollars;
Lot №3 – 50 000 (fifty thousand) U.S. Dollars;
Lot №4 – 35 000 (thirty fivethousand) U.S. Dollars;
for supply, installation and technical support of medical equipment during the last 3 (three) years.
If Bidder is not manufacturer:
If a Bidder is not a manufacturer, but is offering the Goods on behalf of the Manufacturer under Manufacturer's Authorization Form (Section IV, Bidding Forms), the Manufacturer shall demonstrate the above qualifications (i), (ii) (1), and the Bidder shall demonstrate that it has successfully completed at least 2 (two) contracts for supply, installation and technical support of similar medical equipment during last 3 (three) years for the amount for each contract that is similar to or more than:
Lot № 1 –100 000(onehundred thousand) U.S. Dollars;
Lot №2 – 130 000 (one hundred thirty thousand) U.S. Dollars;
Lot №3 – 50 000 (fifty thousand) U.S. Dollars;
Lot №4 – 35 000 (thirty fivethousand) U.S. Dollars.
Bidders may bid for one or several lots, as further defined in the bidding document for complete list of items within each lot only. Partial bids will not be considered and will be rejected as non-responsive. Bidders wishing to offer discounts in case they are awarded more than one contract will be allowed to do so, provided those discounts are included in the Letter of Bid.
3. Bidding will be conducted through the International Competitive Bidding procedures as specified in the World Bank’s Guidelines: Procurement of Goods, Works, and Non-Consulting Services under IBRD Loans and IDA Credits & Grants dated January 2011 (revised in July 2014) (“Procurement Guidelines”), and is open to all eligible bidders as defined in the Procurement Guidelines. In addition, please refer to paragraphs 1.6 and 1.7 setting forth the World Bank’s policy on conflict of interest.
4. Interested eligible bidders may obtain further information from Health care Department of Lviv Oblast State Administration and inspect the bidding documents during office hours 10:00 to 17:00 at the address given below.
5. A complete set of bidding documents in English or Ukrainian may be purchased by interested eligible bidders upon the submission of a written application to the address below and upon payment of a nonrefundable fee of USD 50 or an equivalent amount in Ukrainian Hryvnia. The document will be sent by mail/airmail or handed to the Bidder’s representative at specified address.
6. Bids must be delivered to the address below on or before11:00 local timeon October13, 2016.Electronic bidding will not be permitted.Late bids will be rejected. Bids will be publicly opened in the presence of the bidders’ designated representatives and anyone who choose to attend at the address below.
7. All bids must be accompanied by a Bid Security. The amount and currency of the bid security shall be:
Lot №1– 7 000 (seven thousand) U.S. Dollars or an equivalent amount in the currency of the bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date.
Lot №2– 8 000 (eight thousand) U.S. Dollars or an equivalent amount in the currency of the bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date.
Lot №3– 3 000 (three thousand) U.S. Dollars or an equivalent amount in the currency of the bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date.
Lot №4– 2 000 (two thousand) U.S. Dollars or an equivalent amount in the currency of the bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date.
Bid Security shall be in the form of irrevocable bank guarantee. All other forms of securities are unacceptable by the Purchaser.
