Name of the Contract: Medical equipment for equipping medical ambulatories of Dnipropetrovsk oblast.

1. The Government of Ukraine has received financing from the World Bank toward the cost of the Serving People Improving Health Project, No. 8475-UA , and intends to apply part of the proceeds toward payments under the contract for procurement of medical equipment for equipping medical ambulatories of Dnipropetrovsk oblast. Bidding process will be governed by the World Bank’s rules and procedures.
2. Health Care Department of Dnipropetrovsk oblast state administration now invites sealed bids from eligible bidders for procurement of medical equipment for equipping medical ambulatories of Dnipropetrovsk oblast, for the following items:
- Tonometer with set of cuffs for measuring blood pressure on adults and infants (for measuring arterial blood pressure in arms and legs) – 218 pcs.
- Microstethoscope – 285 pcs.
- Portable oto-ophthalmoscope – 39 pcs.
- Medical measuring tape (roulette) 1.5 m – 442 pcs.
- Device for measuring sugar level in the blood, delivered as set – 719 pcs.
- Height meter – 246 pcs.
- Hospital scale (for adults) – 115 pcs.
- Electrocardiograph – 55 pcs.
3. The Bidders must submit their Bids for the entire list of items that are covered by this tender. Partial bids will not be considered and will be rejected as non-responsive.
4. Bidding will be conducted through the National Competitive Bidding (NCB) procedures as specified in the World Bank’s Guidelines: Procurement of Goods, Works, and Non-Consulting Services under IBRD Loans and IDA Credits & Grants dated January 2011 (revised in July 2014) (“Procurement Guidelines”); and is open to all Eligible Bidders as defined in the Procurement Guidelines. In addition, please refer to paragraphs 1.6 and 1.7 setting forth the World Bank’s policy on conflict of interest.
5. Interested eligible bidders may obtain further information from Health care Department of Lviv Oblast State Administration and inspect the bidding documents during office hours 9:00 to 17:00 at the address (1) given below.
6. A complete set of Bidding Documents in English or Ukrainian may be received by interested bidders for free subject to submission of a written Application to the address (1) below. Authorized representatives of the Bidders may receive the Bidding Documents at the office of Sub-Project Management Unit at the address (1) below or by method of courier delivery upon condition that this service shall be made payable by the appropriate bidder directly to the providing delivery agents.
7. Qualification requirements include: (i) unavailability of overdue debts to the tax authorities; (ii) at least 5 (five) years if experience in manufacturing or supplying similar products; (iii) successful completion of at least 2 (two) contracts for the supply and installation of similar equipment during the previous 5 (five) years for the amount that is not less than:
$130,000.00 (one hundred and thirty thousand) US Dollars.
(Contracts, executed in UAH, shall be re-calculated according to exchange rate, established by the NBU (https://bank.gov.ua/control/uk/curmetal/detail/currency?period=daily currency?period=daily) as of the contract execution date).

8. Bids must be delivered to the address (1) below on or before 11:00 local time on February 26, 2019.
Electronic bidding will not be permitted. Late bids will be rejected. Bids will be publicly opened in the presence of the bidders’ designated representatives and anyone who choose to attend at the address (2) below.
9. Bids must be supplemented with Bid Security. Amount and currency of the Bid Security are as follows:
UAH 100,000.00 (one hundred thousand Ukrainian Hryvnias).
Bidders may offer the Guarantee of equivalent amount in the currency of a participant’s country at the exchange rate of the National bank of Ukraine (www.bank.gov.ua) 14 (fourteen) days prior to the beginning of bidding.
Bank Guarantee must be submitted only in the form of an irrevocable bank guarantee. Any other forms of the bank guarantee shall be unacceptable for Purchaser.
