The World Bank will help women to detect cancer disease

The implementation of the World Bank's "Serving People, Improving Health" Project started in June of the current year in Ukraine. The initial objective of the project is carrying out health care reform in eight oblasts, including the Lviv oblast.
The funds of the World Bank have been distributed during the endorsement of the loan agreement between the government of Ukraine and the International Bank for Reconstruction and Development. Thus, 10.150 million U.S. Dollars were allocated for the Lviv oblast. Most of these funds will be used to purchase diagnostics equipment for Cancer Treatment and Diagnostics Centre, multi-district screening centres, and cytology laboratories.
It is envisaged that the funds of the World Bank allocated for the Lviv oblast will be used to introduce an electronic registry of the female population, which will enable a doctor to promptly define which patient should primarily be invited to undergo examination, and will allow to track the proportion of the screened women in the Lviv oblast. It is planned to implement the introduction of the electronic registry at the primary, secondary, and tertiary levels in medical outpatient units, antenatal clinics, screening centres, and Cancer Treatment and Diagnostics Centre, as well as in cytology laboratory; thus the process of obtaining the results of the examinations and analyses will be optimized.
In the framework of the project, it is also planned to carry out a mass-media campaign to promote early diagnosis and ещ encourage women aged 18-65 years to undergo free examination in order to detect and prevent cervical and breast cancer, as this is considered to be a key to their health. According to the statement made by the Subproject Management Consultant, Ms. Khrystyna Pak, "Without active rooting of the women for their health, the purchased modern diagnostics equipment may fail to meet the anticipated expectations. After all, whatever equipment a medical facility may use, one should keep in mind that should women not undergo the examination, and should they not perceive the understanding of the importance of early diagnosis, the concession in the rates pertaining to the prevention of diseases will not be achieved."
Currently, the consultants have been selected to assist the Health Care Department in the implementation of the subproject. Weekly group meetings of the Project Management Group are held on a regular basis, which allow the consultants and the employees of the Department and doctors to discuss all the actions and measures referred to in the subproject in details. In her commentary for Dyvys.info, the Subproject Management Consultant, Ms. Khrystyna Pak, stated that "The implementation of the system for monitority the screening of cervical and breast pathology in the primary, secondary and tertiary levels will start in the near future. What it takes to carry out this process is the organizational and technical support for multi-district screening centres operation in Chervonohrad, Sambir, Stryi, Drohobych, and Lviv".
It is envisaged to carry out the procurement and distribution of consumables for cytological screening among the centres in the cities and districts, the reorganization of existing oncology departments in multi-district screening centres, and setting up their operation. Later on, the specialists will develop an electronic registry for the exchange of information between the centres that will enhance the quality and effectiveness of cancer treatment, reduction of disability, increase in the level of radical therapy by the approval of the procedure for additional examination and selection of the patients for treatment.
Doctors also plan to put into practice the use of special combined brushes for taking smears and a smear staining method in accordance with liquid-based cytology technique that will increase the efficiency of diagnosis and reduce the number of non-informative smears. It is planned to install equipment for a volumetric mammographic screening by means of digital mammography devices in multi-district centres and in Lviv. Doctors also plan to acquaint themselves with 3D mammography tomosynthesis and stereoscopic biopsy for the diagnosis of early forms of breast cancer that are not palpable.
Ms. Khrystyna Pak stated that "Most of the equipment will be procured through the international competitive bidding under the rules of the World Bank. In particular, it is envisaged to purchase gynaecological chairs, operating tables, colposcopes, microscopes, electrosurgical units, ultrasound devices, and digital mammography devices."
Doctors expect that after the project implementation, the share of newly diagnosed cases of breast cancer in the Lviv oblast, specifically those pertaining to the early stage, will increase by 30%. The number of organ-saving operations on the breast is envisaged to increase up to 25%, and those relating to the cervical cancer - up to 10%.
The project ends in September 2017, and is the shortest in its duration among all the oblasts involved; however hardly the most important objective of the subproject is to set up the operation of preventive oncology so that it may function effectively even upon the completion of the subproject implementation.

Note: The whole world in the first month of the new year focuses on women's health, particularly recalling the need to pass regular examination at gynaecology offices - cervical pathology screening using cytology smear by Papanicolaou stain (Pap test).
