Contract Title:"Specialized Medical Equipment for the Oblast Cardiologic Dispensary and Oblast Hospital”.

Reference no. (as per the Procurement Plan):# ІСВ 7.1.6

1.         The Government of Ukraine has received financing from the World Bank toward the cost of the “Serving People, Improving Health” Project and intends to apply part of the proceeds toward payments under the contract for Specialized equipment for regional hospital and cardiac center.Bidding process will be governed by the World Bank’s rules and procedures.

2.         The Department of Health of Poltava Oblast State Administrationhereby invites sealed bids from eligible bidders for the supply ofspecialized medical equipment for the Oblast Cardiologic Dispensary and Oblast Hospital, namely:

LOT1:

1. MRI scanner — 1 pc.

LOT2:

1. Computed tomography system — 2 pcs

2. Digital X-ray suite (for 3 workplaces) — 1 pc.

3. Digital X-ray suite (for 2 workplaces) — 2 pcs

LOT3:

1. Extracorporeal shockwave myocardial therapy system — 1 pc.

LOT4:

1. Neurosurgical microscope — 1 pc.

2. Navigation system of neurosurgical microscope — 1 pc.

LOT5:

1. Laser ophthalmic photo-coagulator system — yellow-wave laser combined with green-wave — 1 pc.

LOT 6:

1.System for OCT (optic-coherent tomography)-2 pcs

2. Optic fiber catheters for optic-coherent tomography — 400 pcs

3.Fractional flow reserve measurement system-2 pcs

4. Guidewires for fractional flow reserve measurement system — 400 pcs

 

3.       Bidders may bid for one or more lots, as further defined in the bidding document, for the complete list of items within each lot.Partial bids will not be considered and will be rejected as non-responsive.Bidders wishing to offer discounts if awarded multiple contracts will be allowed to do so provided those discounts are included in the Letter of Bid.

4.       Bidding will be conducted through the International Competitive Bidding (ICB) procedures as specified in the World Bank’s Guidelines: Procurement of Goods, Works, and Non-Consulting Services under IBRD Loans and IDA Credits & Grants  dated January 2011 (revised in July 2014) (“Procurement Guidelines”), and is open to all eligible bidders as defined in the Procurement Guidelines.In addition, please refer to paragraphs 1.6 and 1.7 of the Procurement Guidelines setting forth the World Bank’s policy on conflict of interest.

5.       Interested eligible bidders may obtain further information and inspect the bidding documents during office hours 10:00 to 17:00 hoursat the address given below.

6.       A complete set of bidding documents in English or Ukrainian may be obtain after writing request of bidder to the mail below.

The bidding document will be sent by mail to the Bidder’s representative at specified e-mail address.

7. Qualification Requirements:(i) good standing with tax authorities; (ii) at least five (5) years of experience in manufacturing similar goods for each lot; (iii) Successful completion of at least two (2) contracts for supply, installation and technical support of medical equipment during the last five (5) years for the amount for each contract that is similar to or more than:

Lot 1:in amount not less than 1,600,000 US Dollars,

Lot 2:in amount not less than 2,300,000 US Dollars,

Lot 3:in amount not less than 400,000 US Dollars,

Lot 4:in amount not less than800,000 US Dollars,

Lot 5:in amount not less than 80,000 US Dollars,

Lot 6:in amount not less than 300,000 US Dollars.

 

or  1 (one) similar contract for each lot for the amount contract that shall not be less than:

 

Lot 1:in amount not less than 3,200,000 US Dollars,

Lot 2:in amount not less than 4,600,000 US Dollars,

Lot 3:in amount not less than 800,000 US Dollars,

Lot 4:in amount not less than 1,600,000 US Dollars,

Lot 5:in amount not less than 160,000 US Dollars,

Lot 6:in amount not less than 600,000 US Dollars

Qualification of Bidders who submitted Bids for more than one lot will be evaluated on the cumulative basis.

8. Bids must be delivered to the address below by 11:00 local time
on August 15, 2018.Electronic bidding will not be permitted.Late bids will be rejected.Bids will be publicly opened in the presence of the bidders’ designated representatives and anyone who choose to attend at the address below.

9. All bids must be accompanied by a Bid-Securing Declaration.
