Contract Title: Medical equipment for primary health care сenters, central district hospital, city hospitals, regional hospitals
Reference No. (as per Procurement Plan): № ICB 7.1.1
1. The Ukraine has received financing from the World Bank toward the cost of the Serving People Improving Health Project, No.8475-UAand intends to apply part of the proceeds toward payments under the contracts for Medical equipment for primary health care сenters, central district hospital, city hospitals, regional hospitals. Bidding process will be governed by the World Bank’s rules and procedures.
2. The Health Department of Poltava Oblast State Administrationnow invites sealed bids from eligible bidders for the following medical equipment for primary health care сenters, central district hospital, city hospitals, regional hospitals:
LOT1:
1. Stationary electronic tonometer - 68 pcs.
2. Set for blood pressure measurement (a tonometer and a phonendoscope)– 3792 sets.
3. Blood pressure measurement cuffs for legs - 100 pcs.
4. Medicalbags - 1800 pcs.
LOT2:
1. Electronic medical scales - 970 pcs.
2. Height meter - 970 pcs.
3. Veloergometer – 38 pcs.
4. Ophtalmoscope – 982 pcs.
LOT3:
1. 24h arterial blood pressure monitor complete with software and 3 recorders– 40 pcs.
2. 24h arterial blood pressure monitor complete with software– 350 pcs.
3. Holter monitoring system – 46 pcs.
4. Treadmill-based cardiologic rehabilitation system – 1 pc.
5. Cardiac (endocardial) stimulator – 3 pcs.
LOT4:
1. Device for remote ECG transmission - 947 pcs.
2. ECG machine with remote ECG transmission function – 450 pcs.
3. Central stations for receipt of remotely transmitted ECG data from devices–34 pcs.
4. Central stations for receipt of remotedly transmitted ECG data from ECG machines with remote ECG transmission function – 36pcs.
LOT5:
1. Portable ultrasonic scanner with cardiopackage – 36 pcs.
2. Portable ultrasonic expert class apparatuswith a cardiopackage – 4 pcs.
3. Ultrasonic scanner – 37 pcs.
4. Expert class ultrasonic scanner – 4 pcs.
LOT6:
1. Artificial respiration unit – 40 pc.
LOT7:
1. Defibrillator with external stimulation functionality - 462
LOT8:
1. Angiograph – 1 pc.
2. X-ray diagnostics system with digitalimage processing – 1 pc.
3.Injector for injection ofcontrast liquid in course of coronaroventriculography, angiopulomonography and aortography – 1 pc.
LOT9:
1. Navigational and ablative system – 1 pc.
2. Radiofrequency generator– 1 pc.
3. Bidders may bid for one or several lots, as further defined in the bidding document for complete list of items within each lot only. Partial bids will not be considered and will be rejected as non-responsive.Bidders wishing to offer discounts in case they are awarded more than one contract will be allowed to do so, provided those discounts are included in the Letter of Bid.
4. Bidding will be conducted through the International Competitive Bidding procedures as specified in the World Bank’s Guidelines: Procurement of Goods, Works, and Non-Consulting Services under IBRD Loans and IDA Credits & Grants dated January 2011 (revised in July 2014) (“Procurement Guidelines”), and is open to all eligible bidders as defined in the Procurement Guidelines. In addition, please refer to paragraphs 1.6 and 1.7 setting forth the World Bank’s policy on conflict of interest.
5. Interested eligible bidders may obtain further information and inspect the bidding documents during office hours 10:00 to 17:00 hoursat the address given below.
6. Acomplete set of bidding documents in English or Ukrainian may be purchased by interested eligible bidders upon the submission of a written application to the address below and upon payment of a nonrefundable fee of USD 50,00 oran equivalent amount in Ukrainian Hryvnia. The method of payment will be direct transfer to specified banking account:

All banking charges and fees related to the bank transfer shall be for bidder’s account.
The document will be sent by mail/airmail or handed to the Bidder’s representative at specified address.
7. Qualifications requirements include: (i) absence of deferred debts to tax authorities; (ii) At least five (5) years of experience in manufacturing similar goods for each lot; (iii) Successful completion of at least 2 (two) contracts for supply, installation and technical support of medical equipment during last 5(five) years for the amount for each contract that is similar to or more than:
Lot1: in amount not less than 680,000 US Dollars,
Lot2: in amount not less than 400,000 US Dollars
Lot3: in amount not less than 470,000 US Dollars
Lot4: in amount not less than 1,800,000 US Dollars
Lot 5: in amount not less than 5,500,000 US Dollars
Lot 6: in amount not less than 880,000 US Dollars
Lot7: in amount not less than 4,800,000 US Dollars
Lot8: in amount not less than 820,000 US Dollars
Lot9: in amount not less than 450,000 US Dollars
Qualification of the Bidders submitting bids for several lots will be assessed on a cumulative basis.

8. Bids must be delivered to the address below on or before 15:00 local time
on July14th, 2016.Electronic bidding will not be permitted.Late bids will be rejected. Bids will be publicly opened in the presence of the bidders’ designated representatives and anyone who choose to attend at the address below.
9. All bids must be accompanied by a Bid Security. The amount and currency of the bid security shall be:
LOT 1: 10000 (ten thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date.
LOT2: 6 000 (six thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
LOT3: 7 000 (seven thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
LOT 4: 30000 (thirty thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
LOT5: 90 000 (ninety thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
LOT 6: 14000 (fourteen thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
LОТ 7: 78 000 (seventy eight thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
LОТ 8: 13 000 (thirteenthousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date.
