Contract Title:“Procurement of medical devices and diesel generator for Interventional Radiology Unit of MI Sarny Central District Hospital of Sarny District Council as an affiliated branch of Interventional Radiology Unit of MI Rivne Region Clinical Hospital of Rivne Region Council, and services covering equipment installation and staff training”

1. The Government of Ukraine received a loan from the World Bank for funding the project Serving People, Improving Health. A part of the funding shall be allocated for the Contract for the procurement of medical equipment to be installed in PHCC, CDH, city/town and regional hospitals. Bidding will be conducted pursuant to the World Bank rules and procedures.
2.Health Department of Rivne Region State Administration invites sealed bids from eligible bidders for supply of the following items forming part of the Procurement of medical devices and diesel generator for Interventional Radiology Unit of MI Sarny Central District Hospital of Sarny District Council as an affiliated branch of Interventional Radiology Unit of MI Rivne Region Clinical Hospital of Rivne Region Council, and services covering equipment installation and staff training:
LOT 1:
- Angiography system: 1 pc.
- Diesel generator unit: 1 pc.
LOT 2:
- Electrophysiology heart study unit for radiofrequency ablation with a pump and programmable cardiac pacing with a set of consumables: 1 pc.
LOT 3:
- Defibrillator: 4 pcs.
- 12 lead ECG machine: 3 pcs.
- Medical ventilator: 1 pc.
- Patient monitor: 8 pc.
- Cardiac marker analyzer: 1 pc.
- Pacemaker for temporary cardiac pacing: 4 pcs.
- Medical suction machine – aspirator: 3 pc.
- Round-the-clock monitoring station: 2 pcs.
- Syringe pump: 34 pcs.
3. Fractional Bids shall not be considered and shall be rejected as nonresponsive.
4. Bidding will be conducted through the International Competitive Bidding (ICB) procedures as specified in the Guidelines: Procurement of Goods, Works and Non-Consulting Services under IBRD Loans and IDA Credits & Grants by World Bank Borrowers (“Procurement Guidelines”) published in January 2011 (revised in July 2014), and is open to all eligible bidders as defined in the Procurement Guidelines. In addition, please refer to paragraphs 1.6 and 1.7 setting forth the World Bank’s policy on conflict of interest.
5. Interested eligible bidders may obtain further information and inspect the bidding documents during office hours from 10 a.m. to 5 p.m. local time at the address given below.
6. A complete set of bidding documents in English or Ukrainian may be obtained by interested eligible bidders in electronic form free of charge upon submission of a written application to the address below (item 10).
The bidding documents may be obtained by bidders’ representatives free of charge at the Purchaser’s office at the address below (item 10) or by courier, provided that the delivery service is arranged separately and paid by the bidder directly to the delivery agent providing it.
7. Qualification requirements shall include the following:
LOT 1:
(1) at least five (5) years of experience of similar goods production (please provide documents evidencing the date of state registration, economic activities and experience);
(2) successful execution of similar contracts (manufacturing/supply, installation and technical support of medical equipment) over the past five (5) years meeting the following requirements:
– at least two (2) contracts with the amount of each of them equivalent to or exceeding 800,000.00 (eight hundred thousand US dollars).
LOT 2:
(1) at least five (5) years of experience of similar goods production (please provide documents evidencing the date of state registration, economic activities and experience);
(2) successful execution of similar contracts (manufacturing/supply, installation and technical support of medical equipment) over the past five (5) years meeting the following requirements:
– at least two (2) contracts with the amount of each of them equivalent to or exceeding 160,000.00 (one hundred sixty thousand US dollars).
LOT 3:
(1) at least five (5) years of experience of similar goods production (please provide documents evidencing the date of state registration, economic activities and experience);
(2) successful execution of similar contracts (manufacturing/supply, installation and technical support of medical equipment) over the past five (5) years meeting the following requirements:
– at least two (2) contracts with the amount of each of them equivalent to or exceeding 100,000.00 (one hundred thousand US dollars).
The Bidders shall furnish documentary evidence (original bank statements of turnover) that they meet the following financial requirements:
- average annual turnover in calendar years 2015, 2016 and 2017 shall be:
LOT 1: equivalent to or exceeding 2,400,000.00 (two million four hundred thousand US dollars) in each calendar year;
LOT 2: equivalent to or exceeding 480,000.00 (four hundred eighty thousand US dollars) in each calendar year;
LOT 3: equivalent to or exceeding 300,000 (three hundred thousand US dollars) in each calendar year;
As part of the bidding documents, the Bidders are required to furnish documents confirming their Bid's compliance with technical, qualitative, quantitative and other requirements for the procurement item set by the Client.

8. Bids must be delivered to the address below on before November 13, 2018, 11 a.m. local time. Electronic bidding will not be permitted. Late bids will be rejected. Bids will be opened in the presence of the bidders’ representatives who choose to attend at the address below.
9. All bids must be accompanied by a Bid Security. The amount and currency of the bid security shall be:
LOT 1: 12,000 (twelve thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate of the National Bank of Ukraine (http://www.bank.gov.ua) as of day 14 (fourteen) prior to the date of Bid submission.
LOT 2: 7,500 (seven thousand five hundred) US dollars or an equivalent amount in the currency of the Bid at the exchange rate of the National Bank of Ukraine (http://www.bank.gov.ua) as of day 14 (fourteen) prior to the date of Bid submission.
LOT 3: 2,500 (two thousand five hundred) US dollars or an equivalent amount in the currency of the Bid at the exchange rate of the National Bank of Ukraine (http://www.bank.gov.ua) as of day 14 (fourteen) prior to the date of Bid submission.
