Procurement of medical equipment and consumables for 138 existing ambulatories of general practice and family medicine
Reference No. (as per Procurement Plan): № ICB-G 6.2.1.1
1. The Ukraine has received financing from the World Bank toward the cost of the Serving People Improving Health Project, and intends to apply part of the proceeds toward payments under the contracts for Procurement of medical equipment and consumables for 138 existing ambulatories of general practice and family medicine. Bidding process will be governed by the World Bank’s rules and procedures.
2. The Healthcare Department of Volyn Oblast State Administration The Healthcare Department of Volyn Oblast State Administration now invites sealed bids from eligible bidders for the following Procurement of medical equipment and consumables for 138 existing ambulatories of general practice and family medicine:
1. Stethoscope – 414 pcs;
2. Mechanical sphygmomanometer – 414 pcs;
3. Child sphygmomanometer cuff - 414 pcs
4. Femoral sphygmomanometer cuff - 414 pcs
5. Portable ophthalmoscope - 138 pcs;
6. Floor height rod – 138 pcs;
7. Infant scale – 138 pcs;
8. Adult scale – 138 pcs;
9. Portable ECG recorder - 138 pcs;
10. Cooling medical box for biological materials transportation – 138 pcs;
11. Rapid-response urine analyzer - 138 pcs;
12. Reagents for rapid-response urine analyzer – 27600 pcs
13. Biochemical rapid-response blood analyzer – 138 pcs;
14. Reagents for biochemical rapid-response analyzer (glucose) – 27600 pcs;
15. Reagents for biochemical rapid-response analyzer (cholesterol) – 27600 pcs;
16. Reagents for biochemical rapid-response analyzer (triglycerides) – 27600 pcs;
17. Pulseoxymeter – 138 pcs;
18. Nebuliser – 138 pcs;
19. Refrigeration cabinet – 138 pcs;
20. Eye chart Rott apparatus - 138 pcs;
21. Bag-kit for therapist - 138 pcs;
22. Medical couch – 552 pcs;
23. Medicine cabinet with the safe - 138 pcs;
24. Utility table (for instruments) – 138 pcs;
25. Swaddling table - 138 pcs;
26. Gynecological examination chair - 90 pcs
3. Bidders may bid for one or several lots, as further defined in the bidding document for complete list of items within each lot only. Partial bids will not be considered and will be rejected as non-responsive.
4. Bidding will be conducted through the International Competitive Bidding (ICB) procedures as specified in the World Bank’s Guidelines: Procurement of Goods, Works, and Non-Consulting Services under IBRD Loans and IDA Credits & Grants dated January 2011 (revised in July 2014) (“Procurement Guidelines”), and is open to all eligible bidders as defined in the Procurement Guidelines. In addition, please refer to paragraphs 1.6 and 1.7 setting forth the World Bank’s policy on conflict of interest.
5. Interested eligible bidders may obtain further information and inspect the bidding documents during office hours 9:00 to 17:00 hours at the address given below(1).
6. A complete set of bidding documents in English or Ukrainian may be purchased by interested eligible bidders upon the submission of a written application to the address below (1) and upon payment of a nonrefundable fee of USD 6,00 (six) or equivalent ammount in Ukrainian hryvnia at exchange rate on the payment date indicated on the official website of the NBU (https://bank.gov.ua/control/uk/curmetal/detail/ currency?period=daily). Bidders' representatives may obtain bidding documents in the office of the consultation support group at the address below (1) or by a courier provided that delivery service will be separately paid by bidder directly to mail agent that provides it
7. Qualifications requirements include: (i) absence of deferred debts to tax authorities; (ii) At least five (5) years of experience in manufacturing/ supplying similar goods for each lot; (iii) Successful completion of contracts for supply, installation and technical support of medical equipment during last 5 (five) years with the following requirement:
- one contract that is not less than: 700 000.00 (seven hundred thousand US Dollars),
- or two contracts, each of themis not less than:350 000.00 (three hundred fifty thousand USD)
- or three contracts, each of themis not less than:250 000.00 (two hundred fifty thousand USD)

8. Bids must be delivered to the address below (2) before 10:00 local time on December 22, 2016. Electronic bidding will not be permitted. Late bids will be rejected. Bids will be publicly opened in the presence of the bidders’ designated representatives and anyone who choose to attend at the address below.
9. All bids must be accompanied by a Bid Security. The amount and currency of the bid security shall be:
10 000.00 (ten thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the Bids submission date specified in ITB 22.1;
