Contract Title: Procurement of medical diagnostic equipment and consumables
Reference No. (as per Procurement Plan): № ICB 2.1.8
1. The Ukraine has received financing from the World Bank toward the cost of the Serving People Improving Health Project, No.8475-UAand intends to apply part of the proceeds toward payments under the contracts for the following medical diagnostic equipment and consumables. Bidding process will be governed by the World Bank’s rules and procedures.
2. The Health Department of Zaporizhzhia Oblast State Administrationnow invites sealed bids from eligible bidders for the following medical diagnostic equipment and consumables:
LOT 1:
1. Daily blood pressure monitoring apparatus – 100 pcs.
2. Daily electrocardiography registration apparatus – 10 pcs.
LOT2:
1. Apparatus for ultrasound diagnostics with the possibility of examination of the heart and blood vessels – 7 pcs.
LOT3:
1. Biochemical analyzer with consumables – 3 pcs.
2. Immunochemyluminescent analyzers – 3pcs.
LOT4:
1. Insulin pump – 7 pcs.
2. Consumables for insulin pump:
- Infusion system – 420 pcs.
- Reservoirs – 420 pcs.
- Glucose sensor – 210 pcs.
LOT5:
1. Medical Centrifuge – 55 pcs.
2. Consumables for the collection of biological material – 3 pcs.
3. Qualification requirements include
(i) Financial Capability
Local Bidders(participants registered in the Purchaser's country) must provide evidence of the absence of overdue debts to the tax authorities in the form of an original or a notarized copy of the certificate of absence of arrears of taxes, duties (mandatory payments) issued by the relevant State Tax Inspectorate. The certificate should be valid at the time of the opening of the competitive bidding.
(ii) Experience and Technical Capacity
The Bidder shall furnish documentary evidence to demonstrate that it meets the following experience requirement(s):
(1) At least five (5) years of experience in manufacturing similar goods for each lot;
The qualification of the Bidders submitting bids for several lots will be assessed for each lot separately.
(2) Successful completion of at least 2 (two) similar contracts during last 5 (five) yearsfor the amount for each contract:
Lot 1: in amount not less than 55 000 (fifty five thousand) US Dollars
Lot 2: in amount not less than 700000 (seven hundred thousand) US Dollars
Lot 3: in amount not less than 224000 (two hundred and twenty two thousand) US Dollars
Lot 4: in amount not less than 34000 (thirty thre thousand) US Dollars
Lot 5: in amount not less than 83000 (eighty three thousand) US Dollars
Qualification of the Bidders submitting bids for several lots will be assessed on a cumulative basis.
The Bidders should submit in the tender offers the documents confirming the compliance of the bid of the Bidder with the technical, qualitative, quantitative and other requirements to the subject of procurement, established by the Purchaser.

4. Bidders may apply for one or more lots, in accordance with the Bidding Documents, for a complete list of items within each lot. Partial applications will not be considered and will be rejected as non-compliant. Bidders wishing to offer a discount in the event that more than one agreement is concluded with them will have such an opportunity, provided that the respective discounts are taken into account in the application.
5. Bidding will be conducted through the International Competitive Bidding procedures as specified in the World Bank’s Guidelines: Procurement of Goods, Works, and Non-Consulting Services under IBRD Loans and IDA Credits & Grants dated January 2011 (revised in July 2014) (“Procurement Guidelines”), and is open to all eligible bidders as defined in the Procurement Guidelines. In addition, please refer to paragraphs 1.6 and 1.7 setting forth the World Bank’s policy on conflict of interest.
6. Interested eligible bidders may obtain further information and inspect the bidding documents during office hours 10 a.m. to 5 p.m. at the address given below.
7. A complete set of bidding documents in English or Ukrainian may be purchased by interested eligible bidders upon the submission of a written application to the address below and upon payment of a nonrefundable fee of USD 50.00 (fifty) oran equivalent amount in Ukrainian Hryvnia at the exchange rate of the National Bank of Ukraine (www.bank.gov.ua) on the date of payment. The method of payment will be direct transfer to specified banking account:

The document will be sent by mail/airmail or handed to the Bidder’s representative at specified address.
8. Bids must be delivered to the address below on or before 11:00 a.m. local time
onMay 7, 2018. Electronic bidding will not be permitted.Late bids will be rejected. Bids will be publicly opened in the presence of the bidders’ designated representatives and anyone who choose to attend at the address below at 11:00 a.m.local time on May 7, 2018.
9. All bids must be accompanied by a Bid Security. The amount and currency of the bid security shall be:
LOT 1: 1 000 (one thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date.
LOT 2: 13 000 (thirteen thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
LOT 3: 3 000 (three thousand) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
LOT 4: 600 (six hundred) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
LOT 5: 1 500 (one thousand five hundred) US dollars or an equivalent amount in the currency of the Bid at the exchange rate indicated on the Financial Times website (www.ft.com) on the date 14 (fourteen) days before the bid submission date;
Bid Security shall be in the form of irrevocable bank guarantee. All other forms of securities are unacceptable by the Purchaser.
